Hockey NSW Athlete Release Form

This form is to be completed by any Hockey NSW team player who wishes to request a release to not travel with their Hockey NSW team by the arranged travel method under the care of the appointed Hockey NSW team officials.

Arrival at a Championship
Upon arrangement with the Team Manager, the player will be expected to arrive at the championship location by the agreed time prior to the commencement of the championship.

Release from a championship
The player will be released following the conclusion of the final day of the championship at the agreed time as determined by team management.

Any player granted approval to travel to the championship by their own method or granted a release at the end of the championship must understand that Hockey NSW and the appointed Hockey NSW team officials will cease all responsibility and duty of care over the player before the agreed arrival time and/or after the agreed release time.
